The type of client case assessment offered via this complimentary service form is general or direct case-specific professional guidance and assessment is provided for public and member’s clients.

Documents to attach:

• You may attach your or your co applicant’s resumés,
• Attach copy of any correspondence from Immigration office or court or Government office, if you have already submitted an application and received a response.
• Attach copy of any correspondence from your current or previous counsel (if applicable).

Documents NOT required:
• Do not send any Government ID-related document with this form, such as ID card, passport copy or any such document.
• Do not send any document or information if it is not your own.
